ANNOUNCEMENT OF RETIREMENT

June 1, 2026

To My Patients and Friends,

After 48 years of practicing medicine, I have decided to retire and will be closing my medical practice effective 7/1/26. Making this decision has not been easy. It has been one of the greatest privileges of my life to serve you and your family. I am deeply grateful for the trust you have placed in me throughout the years. The relationships I have built with my patients have been the most rewarding part of my career, and I will always cherish the opportunity to have been part of your healthcare journey.

To ensure continuity of care, I encourage you to establish care with another physician as soon as possible. Upon your written authorization, a copy of your medical records can be sent directly to your new healthcare provider. Your medical records will continue to be maintained in accordance with state and federal requirements. If you would like a copy of your records or would like your records transferred to another provider, please submit a written request. All information regarding the storage facility that will be maintaining your records will be made available on our website. All records will be available through June 1, 2033.

Thank you for allowing me to participate in your care. It has been an honor and a privilege to serve this community. I wish you and your loved ones continued health, happiness, and well-being in the years ahead.

Patients may contact VeriTrust for medical records requests by providing a completed Release for Medical Records Form and paying the appropriate fees.

• Clinics Pages (Search (includes 1-500)): $25.00

• Clinics Pages (500+): $25.00 flat fee

• Electronic Records Pages (Search/Retrieval): $52.12

• Electronic Records Pages (1-24): $1.76 per page

• Electronic Records Pages (25+): Non-billable
